Offshore DROPS: From Findings to Risk Control

In July 2026, a safety alert reported the release of two riser buoyancy modules during an offshore operation. Each component, weighing approximately 28 kg, fell nearly 21 meters and struck the drill floor with enough force to cause a fatality. Fortunately, no one was in the impact zone, although personnel had occupied the area less than a minute before the second object fell.

Having supported inspections and integrity programs across industrial assets, I have found that dropped object incidents rarely stem from a single technical deficiency. More often, they result from the accumulation of small gaps in inspection, maintenance, quality control, and verification processes that, collectively, create the conditions for a serious event.

The investigation behind this incident reinforces that point. Torque verification records were incomplete, retorque requirements had not been fully implemented, refurbishment activities did not fully comply with manufacturer specifications, and acceptance controls failed to identify the deficiencies. Similar events had also been reported on other units.

This case highlights a recurring challenge across the industry: preventing dropped objects is about far more than identifying loose components. The real challenge is ensuring that inspection, maintenance, mechanical integrity, supplier oversight, and corrective action management operate as part of a single risk control framework.

In offshore environments, equipment is constantly exposed to vibration, corrosion, harsh weather conditions, and operational interventions. As a result, an effective DROPS inspection should evaluate not only the presence of objects at height, but also the condition and effectiveness of the barriers designed to prevent them from falling, including fixings, secondary retention systems, supports, and bolted connections.

Equally important, the process should not end with the issuance of an inspection report. The true value lies in converting findings into effective actions, prioritized according to risk, tracked through completion, and independently verified when necessary.

In dropped object prevention, finding the hazard is important. Demonstrating that it remains controlled is what makes a system truly reliable.
